Enrollment forms
Forms marked Required must be completed before your child's first day. Optional forms can be submitted at any time.
Enrollment Application
Complete this form to begin the enrollment process for your child. Includes basic family information, child details, and program selection.
Admission
Provide your child's health history, immunization records, physician contact, and any medical conditions or allergies we should know about.
Child Care Examination Certificate
List authorized pickup contacts, emergency contacts, and any individuals who are NOT permitted to pick up your child.
Child Pickup Consent
Review and sign our tuition schedule, payment due dates, late fee policy, and withdrawal notice requirements.
Photo & Media Release
Grant or decline permission for Mini Melodies to use photos or videos of your child for classroom displays, newsletters, and our website.
Daycare Medical Consent
Annual permission form allowing your child to participate in scheduled field trips and off-site activities throughout the program year.
Discipline Policy
Permission for staff to apply sunscreen or lotion to your child during outdoor activities and field trips.
Daily Routine
Required if your child needs prescription or over-the-counter medication administered during care hours. Includes dosage and schedule.
Hazard Plan
For children with known food or environmental allergies. Outlines symptoms, emergency response steps, and EpiPen authorization if needed.
Partnership Agreement
Helps our team understand and support children with individualized care needs, developmental considerations, or IEP/504 accommodations.
Keep Your Child Home If...
A helpful guide outlining symptoms and situations when your child should stay home to protect the health of all children and staff.
Transportation Agreement
Authorization for your child to be transported to and from school or off-site activities by Mini Melodies staff or approved vehicles.
Verification of Receipt
Confirms you have received and reviewed the Mini Melodies Parent Handbook, including policies on attendance, behavior, and communication.
Things You Need
Submit this form when you plan to withdraw your child from the program. Please provide the required advance notice per your enrollment agreement.
